New Customer Forms

Because Roseco is a wholesale company selling strictly to the jewelry trade industry, if you are a retailer, jeweler, or craftsman and wish to establish a buying account with Roseco, you will need to fill out some of the forms listed below. Not all forms are required! Just fill out the ones appropriate to you.

Credit Card Account Form
604 KB

Fill out this form if you wish to pay by credit card. Credit card customers can also opt to pay with cash or COD cashier's check/money order (pickup orders only).

Credit Application
535 KB

Fill out this form if you wish to pay by check and/or establish an open credit account. This form is meant for established businesses that can provide jewelry trade industry references.

Texas Resale Certificate
25 KB

If your business is located in Texas, please also fill out and sign this resale certificate along with either the credit card account form or the credit application.

You can type your information on the forms. Once we receive your forms, we will process the information and contact you with your new Roseco account number or with any questions we may have.

Note: Existing accounts with no activity for over a year become "inactive" and may require refilling out the forms above to reactivate.

Fax or Email Order Form

If you are an established Roseco customer and wish to fax in or email your order, you can use our convenient order form. Simply fill out the form, save it to your computer, and email it as file attachment … or print and fax it.

Note: There is a required order minimum of $25 (before any tax and ship costs) for orders being shipped/couriered and/or paid by credit card. Also, online-only offers do not apply.

Texas Online Tax Registration

As a wholesale supplier, Roseco requires a resale tax number to establish a customer account. If you do not have a resale tax number, you can obtain one from your state's comptroller office.

If you are located in Texas, you can use the link above to go to the official Texas Comptroller site to apply for one. This tax number is required on the Texas Resale Certificate form.
